
The mission of the Outreach Department is to link seniors to supports and services which enable them to remain independent in their community. We like to think of Outreach as a Core Service of the COA, consisting primarily of information and referral services for seniors and their family members. Outreach staff also function as advocates for clients to assist them in interacting with other agencies. We intervene when clients come to the attention of others including family, neighbors, police, fire and the health department. Outreach serves as a bridge to agencies and services, including:
- Elder Services
- Cape Cod Hoarding Task Force
- SNAP (food stamps)
- Housing
- Homecare
- SHINE (health insurance)
- Fuel Assistance
- Lower Cape Outreach Council
- Holiday Programs
Direct services include:
- Medical Rides
- Telephone Reassurance
- Friendly Visitors.
- Brown Bag and Government Food programs
- The Caleb Chase Fund
- “Buried In Treasures” Hoarding Support Group
Outreach programs for recreation and socialization include:
- Weekly Cribbage
- Monthly Men’s Breakfast and Book Group.
- Weekly Bicycle Group (Seasonal)
