2023 Community Preservation Act Project Funding Request -- Additional Information Submission

Please use this form to upload additional information needed to support an application for Community Preservation Act funds submitted for the October 2023 submission deadline.  If you have more than one file to submit, please fill this form out for each file.

Material must be in a Portable Document Format (pdf) file, not to exceed 5MB.  Files exceeding 5MB must be submitted on a USB memory stick with hardcopy submission.

Supplemental material must also be submitted in five (5) paper copies to:

Supplemental materials not available with the original application must be submitted no later than 4 PM on Friday, December 1, 2023.
